| General Notes | SWITP is an alternative to a person being stuck into a cubicle where they fit into a program rather than their being placed from looking at their interests and individuality. SWITP coordinates service delivery, conducts reciprocal assessment of referral and interagency training, evaluation and funding. The guiding principles are to focus on the individual where family and consumer are equal partners, and to eliminate barriers to planning, reduce duplicate services, give incentives to local teams to communicate and document services, and to conduct interagency cross training.
